• payment, billing, insurer, Medicare, referral or claim information where relevant;

• communications and enquiries you send to us; and

The Life In Motion Physio website does not currently provide a public enquiry form for submitting personal or health information. Please do not send detailed or urgent health information by ordinary email.

• directly from you during an appointment or when you contact us;
• from UMC North Lakes reception or an appointment platform used to arrange your booking;
• from a referring practitioner, another health professional, insurer, claim manager or representative, where authorised or permitted by law;
• through clinical, booking, billing and communication systems used to provide your care; and
• automatically when you use our website, through cookies and analytics or advertising-measurement technologies.
Where practicable, you may make a general enquiry without identifying yourself. We may, however, need accurate identifying and health information to safely arrange or provide healthcare.
4. Online bookings and third-party services
Online booking links on our website take you to Healthengine, a third-party booking service. Information you enter on Healthengine is collected and handled by Healthengine and may be shared with the selected health professional and practice to arrange and manage your appointment.
UMC North Lakes reception answers telephone calls and assists with appointment scheduling for Life In Motion Physio. Information provided to reception for this purpose may be shared with Life In Motion Physio as needed to manage your appointment and provide care.

5. Why we collect, use and disclose information
We may handle personal information to:
• respond to enquiries and arrange appointments;
• assess, provide and coordinate physiotherapy care;

Our website uses Google Analytics 4 and may use Google Ads conversion measurement. These services help us understand general website usage and whether actions such as clicking a booking or telephone link followed an advertisement.

We do not intentionally send names, contact details, appointment details or medical information to Google Analytics or Google Ads.

Where necessary for the purposes described above, and where authorised or permitted by law, information may be disclosed to:

We do not sell personal or health information.
8. Overseas handling
Some technology providers may process or store information outside Australia. Google operates servers around the world. Healthengine states that some of its service providers may handle information in jurisdictions including the United States, the European Union and the Philippines.

9. Storage, security and retention
We take reasonable steps to protect personal information from misuse, interference, loss and unauthorised access, modification or disclosure. Information may be held in physical records and in clinical, booking, billing, email or other electronic systems protected by appropriate access controls and security measures.
We retain information for as long as reasonably needed to provide services and meet clinical, professional, legal, insurance and record-keeping requirements. When information is no longer required, we take reasonable steps to securely destroy it or de-identify it where permitted.
No method of electronic storage or transmission is completely secure. If you believe information has been compromised, please contact us promptly.

You may request access to personal information we hold about you or ask us to correct information that is inaccurate, out of date, incomplete, irrelevant or misleading.
